If you don't have a 7.1 surround system, you won't hear any sound effects (drum / guitar key sounds). In this case, you should turn on `GitaDora Two Channel Audio` option which downmixes everything to stereo. If you need to adjust the volume ratio between BGM vs. sound effects, go to the test menu and adjust volume level. Avoid using 2-channel patches if possible as they often result in buzzing noise.
